You enter the Form 1099-R Exactly as received. Once you finish the 1099-R entry you will be asked a series of questions concerning the withdrawal. A couple of those questions will concern Covid-19 and how you want to report the withdrawal.

You enter the 1099-R and select that it was COVID related distributions and answer the questions. The 3 year repayment is done the the 8915-E form by TurboTax.
