If your state offers the Renters credit you will be prompted to enter your rent information when you complete your stat return, I believe Arizona is one of the States that do offer the credit. Here is a link that explains how it works.

I could not find the form that you need, but here are the phone numbers for the Arizona State Department of Revenue.
