Here are the steps ( in MAC desktop):

Open up the program if you have not already done so.
Switch to Forms mode
In the left pane, scroll down and select Schedule K-1 (if you don't see it, click Open Form at the top).
From the top, select Forms drop-down menu, select Remove [form name].
Follow any on-screen instructions to remove the form or forms.
