How do I remove the Child Care Credit Wks from state return?

I am trying to finish my state taxes, but I keep getting an error when trying to fix the Child Care Credit form, saying: Refundable: Care Provider ID-1 must be provided. I do not have the provider ID and would just like to delete the form all together. How can I do this?
