The state form is asking for "Real Property Taxes paid". Is this a combination of School Taxes (Sept School Tax bill) and County Taxes (January Tax bill)?

Yes. Total real property taxes paid is calculated based on the market value of the property plus, fees for services and the cost of local public schools.
