The medical expense reported on the federal tax return will not flow to the MA state tax return if you used the Standard Deduction on your federal tax return. Look at your federal tax return Form 1040 Line 12.

Standard deductions for 2022

Single - $12,950 add $1,750 if age 65 or older
Married Filing Separately - $12,950 add $1,400 if age 65 or older
Married Filing Jointly - $25,900 add $1,400 for each spouse age 65 or older
Head of Household - $19,400 add $1,750 if age 65 or older

For 2022, if you itemize on U.S. Schedule A (Line 4) and have medical/dental expenses greater than 7.5% of federal AGI, you may claim a medical and dental exemption in Massachusetts equal to the amount you reported on U.S. Schedule A, line 4

If you claimed Medical Expenses on your Federal return, and used Itemized Deductions, you can claim an equal amount in Massachusetts.
