&lt;LI&gt;Go into &lt;I&gt;Forms Mode&lt;/I&gt; by clicking the &lt;B&gt;Forms &lt;/B&gt;icon at the top right of your screen.&lt;/LI&gt;
&lt;LI&gt;In the&lt;I&gt; Forms in My Return&lt;/I&gt; list, scroll down and click on &lt;B&gt;Health LTC Wks&lt;/B&gt;.&amp;nbsp; The form will appear on the screen.&lt;/LI&gt;
