The page is numbered as 180104CH11555 and is titled below as "2018 Voluntary Contributions Schedule."

I was checking and found this page with only one amount given and all the rest are 0's. I went to my list of those in my check book and NONE of them are mine. My list is far longer and does not compare. I'm rather sure that if this were correct I would have signed different and received a larger refund.

I can only give wht I have from the print of my 218 Voluntary lCtribution Schedule Colorado Department of Revenue 180104CH11555

There is no list for the Federal Report.. I assume this is because the Turbo Tax preparation did not have any data in the list. ave. I don't have any idea where the Colorado list came from but it is not mine and Turbo did all of the work. Also, there is a note of a Return Adjustment of the Return.
