One source of confusion is in different versions of the 40-V for 2021. I've seen at least 3 so far. OR Dept Revenue mailed a single Form OR-40-V with a letter dated 4Feb2021 indicating one could copy it if more were needed, or more could be generated from a Revenue Online account www.oregon.gov/dor and clicking on Revenue Online. (In previous years, they would send 4.) The single form indicates it should be mailed to Oregon Dept of Revenue

All the other forms I've seen, including the one in TT State, use the address listed by AnnetteB6, which may be a more current address. Either one will get your payment to DOR, although it may take another day or so for them to shuffle it to the right processing group.
