Payer State ID number

I have state tax withheld on a 1099-R for the state of Utah. The state requires a payer's state ID number that is at least 14 characters long. The 1099-R from TD Ameritrade isn't correct. Can you give me the correct number?

Please contact TD Ameritrade to get the correct number and possibly a corrected 1099R.
