      <title>W-2 Missing a State and State ID</title>

      <description>&lt;P&gt;I got a W-2 from Omaha Life Insurance for short term disability when I was on maternity leave. Unfortunetly they didnt take state taxes out of it and therefore theW-2 has nothing in boxes 15 for Sate and Employer State ID Number but there is an amount in box 16. TurboTax requires that I provide data in the two boxes for 15 so what should I enter here? Or should i just not enter anything in boxes 15 and 16?&lt;/P&gt;&lt;P&gt;&amp;nbsp;&lt;/P&gt;&lt;P&gt;Thanks!&lt;/P&gt;</description>

      <description>&lt;P&gt;Yes, that is correct. Your state return will still include this as income. &amp;nbsp;But without leaving boxes 15-17 blank, it will be impossible to e-file.&lt;/P&gt;</description>
