W-2G and state reporting during a year in which I moved

I received W-2G from Las Vegas - the W-2G have box 14 populated with sate winnings, however no information was provided in box 13 - State/payers state identification number.

Turbo Tax has identified a problem with the information entered due to lack of state information.

1) Should the state information be listed as Nevada with the federal Identification Number ?

2) should the sate be listed as the state in which I reside with the federal identification number??

3) Do I not enter anything in boxes 13-18 despite the value being shown in box 14?

Thank you in advance for your help - I have not found this scenario in my searching so far

If there were no withholdings for the state, you should change the Box 14 entry to blank (using the drop down) so that you can file your return.
