I must supply requested information to the IRS per their 12C letter. I do not want to fax it and mailing is an option. It says to mail to the address at the top of the first page of the letter. All that is there (besides my own address) in top left corner of the first page is:

IRS Internal Revenue Service

OSC

Ogden, UT [removed]

Is that a valid return address for the IRS?

@ddcnga , the Ogden, UT service center address is :

Department of the Treasury
Internal Revenue Service
Ogden, UT 84201

There is often a zip code extension for some specific forms/information request. The "OSC" stands for Ogden Service Center.

I think your CP-12 is legitimate and therefore the return address is legit.

You can also visit your local IRS office ( usually in the local federal building ) to confirm.
