You must have your social security number (or your spouse’s social security number if it is a joint return and your spouse’s number is entered first on the return) and the exact amount of the refund request as reported on your Connecticut income tax return.

Enter the whole dollar amount of the refund you requested. (Do not include cents.) For example, if you requested a refund of $375, enter 375.
