Hi,

I'm currently in the process of filling out form 4506t to request my tax return transcripts, and I have a couple of questions:

I filed jointly from 2016 to 2021. Should I include my ex-husband's name in the form for those years?

The form provides space for only four years. Can I include two forms in one envelope?

Thanks in advance.

I filed jointly from 2016 to 2021. Should I include my ex-husband's name in the form for those years?

yes see line 2a also his Social Security number. note 2016 may not be available any longer.

if you changed your name also sign your current name

The form provides space for only four years. Can I include two forms in one envelope?

to be on the safe side mail separately also mail using a tracking service.

You're not reading the notations on the Form 4506.

Enter the name of the person listed first, then the spouse (probably, you) for a joint return.

If you filed single in later years, you would submit another form.

The form has space to request eight years not four.

@madeleine30

Is it possible to get tax return copies from as far back as 15 years from the IRS? I know that the online transcripts are various times that are available do not go back that far:
