NJ 1040-V Form

I have to file 2022 amended returns for federal and NJ State because an error I made in inputting a 1099 dividend amount. I was able to file the federal amended return electronically and use my checking account to pay the additional amount owed.

I have to mail the NJ amended return and I'll include a check for the amount due. The instructions say to include a payment voucher (Form NJ-1040-V) with my payment. Turbotax doesn't print it out. When I search forms, it's not there.

Where is the form? Thanks.

I was able to find the 2022 NJ-1040-V Form online, however, the mailing address is different from the address on the Turbotax instructions. Which 1 is correct? Thank you,

The Form NJ 1040-V should have been for the taxes owed on the original tax return. You only need to send a check for the taxes owed with the amended state tax return.
