What state, and what city or municipality?

Is the first sentence of your question a quote from a letter that you received? Who is the letter from?

Is it true that you did not file a municipal income tax return for the indicated year or years? Were you required to file either a municipal income tax return or an exemption certificate?

What does the letter say you should do? Read the entire letter carefully.

City Tax Returns

TurboTax only prepares a few city or local tax returns as an extra step in your state return. For most local tax returns you are on your own. Your only record of paying your local tax will be your own bank/credit card records

Getting that notice is a very common thing. Cleveland is a city with a city (municipal) income tax. If you are a resident, you are required to file an annual return. The Central Collection Agency (CCA) administers Cleveland's (and some other cities) tax.

To verify that you live in a city with an income tax enter your address at:

TurboTax can prepare a CCA/Cleveland tax return.

TurboTax (TT) does not automatically prepare a city return, you must initiate it. At the screen, in the Ohio program, titled
"A few things before we wrap up your state taxes"
      scroll down to Ohio City taxes.

    -Scroll down to Ohio City taxes.

TT cannot e-file a CCA return. It must be printed and mailed.

I actually find it easier to fill out a city return by hand or at the CCA web site, than to use TT. Things don't happen automatically and you need to know what you’re doing. The actual RITA/city forms are easier to follow than the TT interview. You can e-file at the CCA site
