My State taxes keep getting rejected for “OH-PIT-004”. I cant figure out what I'm missing.

“Ohio and/or school district tax withheld is claimed on the return(s) but no income R statement forms with Ohio and/or tax withheld are included with the return.”

Can anyone help?

This indicates that you had school district withholding on your W-2, but did not have a SD-100 on your state return.

To add the SD-100 to your return, please follow the instructions below:

Open your return.
Click on State on the left hand side.
Click Edit next to Ohio.
Continue through the state questions until you get to a page that is titled A few things before we wrap up your state taxes.
Scroll down on this page until you see School District Taxes.
Click on Start and follow the on-screen instructions.

Once complete, you can refile your return.

I keep getting this message as well, and I have filled out the SD100, I don't understand why this isn't working. OH-PIT-004 - Ohio and/or school district tax withheld is claimed on the return(s) but no income statement forms with Ohio and/or tax withheld are included with the return. Please correct and retransmit
