Form 804 isn’t an available TurboTax form, but you can upload it into TurboTax if you have a paid TurboTax Online product, or add PLUS to Free Edition.

The document is uploaded on Other Documents but the system keeps asking to upload the PDF on the state return and there is no option to upload it again.

You should try deleting the document and uploading it again. If this doesn't fix your problem, clear your cache. This is to remove these temporary files. A full or corrupted cache can cause problems in TurboTax. You can try these steps if you are using the online version:

Clearing your Cache and Cookies
Using a different browser
Opening an Incognito Window
