I am trying get a specific answer to this question.

I am filing 1040X (using most recent form from Jan 2019). Along with 1040x, I also need to file form 6781 and schedule D to show the changes.

Question is: Do I use 6781 and Schedule D from 2015 (I will have to search for it online) or do I use the latest versions (which are readily available on IRS website)?

You need to use the one appropriate for the tax year you are amending, in case there have been changes in the intervening time.
