If the bank account you used for the 2019 federal refund direct deposit is closed, the IRS will mail a check for the stimulus payment to the address on the 2019 tax return.

The IRS will use the same banking information for the 2nd stimulus that they used for the first one. If that account (or card) is closed then the stimulus will go back to the IRS and they will mail you a check.

My mailing address has also changed. The 1040 form referred to in your answer, is that part of the regular Turbo Tax filing? Or do I need to request a special form or something?

The 1040 is part of your return. No extra form needed.
