 &lt;LI&gt;&lt;SPAN style="font-size:11.25pt"&gt;&lt;SPAN style="font-family:Slack-Lato"&gt;Press 2 for the option of " have not yet received your card".&lt;/SPAN&gt;&lt;/SPAN&gt;&lt;/LI&gt;
 &lt;LI&gt;&lt;SPAN style="font-size:11.25pt"&gt;&lt;SPAN style="font-family:Slack-Lato"&gt;Next you will be prompted to enter your social security number, zip code, and birth date.&amp;nbsp;&lt;/SPAN&gt;&lt;/SPAN&gt;&lt;/LI&gt;
