The IRS is asking you to provide an amended tax return Form 1040X and Form 8962, Premium Tax Credit.

A Form 8962 is filed with the federal tax return if you had health insurance coverage through the Health Insurance Marketplace and received a Form 1095-A for that insurance. Form 8962 is automatically generated by the TurboTax program when you enter the Form 1095-A in the Health Insurance section of the program.
