Each of these forms has a different purpose and which form you use depends on specific details on the nature of the debt owed. &lt;/P&gt;&lt;P&gt;I have attached the forms and instructions to this message as well as a &lt;B&gt;TurboTax FAQ &lt;/B&gt;that explains the difference between the two and which one would suit you best.
