For your HSA contributions, all you need to do is enter your W-2. When you go through the HSA interview, do not enter contributions there unless you made contributions outside of work. Form 5498SA is for your tax records and does not get entered into TurboTax. The 5498-SA often does not match the code W amount in box 12 on the W-2. Your W-2 usually has contributions from you and from your employer.

Form 5498-SA reports contributions to your Health Savings Account (HSA), Archer MSA, or Medicare Advantage MSA. You don't enter this form into TurboTax; your account custodian sends it to you for informational purposes only.
