your medical plan does not make HSA contributions. the tax laws only allow the taxpayer and/or employer. any HSA contributions would be in box 12 with a W code. your medical plan may require FSA contributions by you and/or your employer. this is completely different.

A question regarding entering contributions to my HSA. I did not make contributions to my HSA and looking at my Leave and Earnings statements can not find that my employer did either. Base on this, believe my medical plan must. Did question in Turbo Tax is did I or my employer on my behalf make contributions. Do I enter contributions made by my medical plan?

Thanks for the quick reply as I've wait to the last minute. I'm assuming you mean box 12 of my W-2. Only entries on box 12 of my W-2 are D and DD.
