&lt;P&gt;&lt;SPAN style="font-family:Arial, Helvetica, sans-serif;font-size:14px;"&gt;3. the care is provided according to a &lt;I&gt;&lt;U&gt;written care plan&lt;/U&gt;&lt;/I&gt; that is prepared by a qualified medical professional or social worker that is reviewed and updated at least once a year.&lt;/SPAN&gt;&lt;/P&gt;</description>
