Where do I put the information from my FTB 3895 form I received?

My tax return was rejected because I did not include necessary pertinent info (specifically from 1095-A and FTB 3895). I figured out how to fill out the 1095-A, but where can I input the information from FTB 3895? Which form do I fill out online? Thanks

To file form from FTB 3895

Open CA return.
Continue until you get to the screen Did you all have health insurance coverage in 2021?
On the next screen, Select Yes to the question Was anyone enrolled in any of these less common plans in 2021?

Once you enter form 3895, TurboTax will generate form 3849.

Form 3849 (Premium Assistance Subsidy) is used to reconcile advance premium assistance subsidy payments.
