If this is for a Form 1099-G from the state of Florida showing your unemployment benefits, the EIN should be 36-4706134.

This will go with the Payer's name & Address:

Special Payments Unit PO BOX 5350 Tallahassee, FL 32314-5350

No, it's a W-2.

If it is a Florida employer and you are referring to the Employer's State ID Number in Box 15, you can leave it blank. There should be no information for state wages or state tax withheld.

If you leave Boxes 15-17 blank, TurboTax may ask you to verify that there is no information is required, but it will not flag it as Needs Review after you confirm that those entries should be blank.
