What is the correct CVS format for crypto reporting?

I used Koinly to generate my CVS file for Turbotax and I think it's missing some things because I am asked to review almost all of the 500 entries.
It usually asks: How did you receive this investment?

I answer: I purchased it

Then it gives me 2 options: I paid sales expenses that aren't included in the sale proceeds reported on the form

None of these apply (I check this one)

The Koinly CVS layout:

Amount, Currency Name, Purchase Date, Date sold, Cost Basis, Proceeds

The columns should be:

Service name (examples: Coinbase)
Asset name (examples: Bitcoin, Ethereum)
Date of purchase
Cost basis
Date of sale
Sale proceeds
