If 14 is blank leave boxes 13 thru 18 all blank.

If you were gambling in a state with no tax, then box 12 would be the last thing on your form, click continue and move on. However, you said there is nothing in box 14, so I am going to assume that you have a state and ID number. Then state winnings in box 14 would match the income in box 1.

Ok got it. Let me ask you this… Do I have to do a state tax form for Nebraska and Iowa? I only one $1000 in Iowa and I paid $50 in taxes

If you only made $1000 you do not have to file in Iowa.

You don't give enough information on Nebraska to be able to answer the question.
