If you don't have anything on your W-2, and you've tried to delete the form. I suggest that you go through the program interview to clear it out.

Stat with:

Deductions and Credits
Scroll to You and Your Family
Select Adoption Credit
If there is an entry here there will be a trash can on the right hand side to clear the entries.

I'd also like to confirm your process to delete a form.

On the menu bar on the left that shows.
1. My Info
2. Federal
3. State
4. Review
5. File
Select Tax Tools
On the drop down select Tools
On the Pop-Up menu select Delete a Form
This will give you all of the forms in your return
Scroll down to the form you want to delete
Select the Form
Click on Delete.

Always use extreme caution when deleting from your tax return. There could be unintended consequences.

If you're not claiming the Adoption Credit and you want to remove the adoption worksheet(s), you must also delete Form 8839: Qualified Adoption Expenses. When you delete form 8889, the instructions will ask if you want to remove related worksheets and schedules. This will remove the adoption worksheet.
