Form 8936 incorrect

Hi I am working on form 8936. There are some issues：

1. part of the text is outside the box in field 1

2. Field 10 is filled but my vehicle is for personal use

3. Field 16 and 17 are empty.

Are those three issues problematic?

I am not sure that I see a problem.

Line 10 in Part II of the 8936 is pre-printed on the form with 2,500 in both columns (see the IRS form here), so you can safely ignore that line.

Do you believe that the amount in lines 15 and 18 are correct? This is what is going on your tax return as a credit.
