      <description>This was dividends from MetLife policy my dad had before he passed away it was not taxed to me nor was I the beneficiary of this .I had to apply for the unclaimed funds from the state of New York because no one else was doing it in my family and didn't want to deal with it...so I did it...and then I got a 1099 – B form in the mail And two boxes are checked on the form number five and number six and number five says if this box is checked it is non-covered security and box&amp;nbsp;&amp;nbsp;number six says reported to IRS gross proceeds..</description>
