Claim for Refund Replacement Check

My e-file was submitted on Feb 24 2020

But I didn't receive check and any form

I don't know why it is delayed or rejected

so I'd like to submit 'Claim for Refund Replacement Check' form

but I've face triviality

Look at the picture, there are blank

what do I have to filled out ?

postcode? or other? I couldn't understand

If someone solve this problem please let me know what can I do

You need to follow the instructions in Where's my refund. If you should file this claim, it is asking on the form for the number you live at on your street. For example: Address is 122 Oak. Your number is 122.

If your return was rejected, you won't get a refund and need to correct the issue.
If your return was accepted, did you check Where's my refund to ensure that you are getting one?
If somebody has a claim against your refund, you won't get it. Student loans, child support, etc. The Treasury Offset Program takes money before it goes to you.
