Form 4825 options not showing up.

My previous employer basically dropped off the face of the Earth and I know I'm not getting my W-2. I need to fill out form 4825, I follow the instructions on the website, get to the section where I'm asked if any "uncommon situations" apply where I am instructed to check something that says "Substitute form- Employer never sent W-2" or something along those lines and there's no option like that. I'm legitimately following these instructions as closely as I can.

This is actually a 4852 that you need to file. The best way to get there is to go to:

tax tools (in the lower left hand corner of your screen)
tools
topic search
Type in 4852 in the search bar

This should be filled out accurately as much as possible. Hopefully, you will have your year-end earnings reported on a paystub or other earnings statement. You will not be able to electronically file with a 4852 so you will need to print and mail your return. Turbo Tax will give you complete instructions regarding the filing and mailing instructions.
