      <description>&lt;P&gt;Your license has an issue date on it. Enter the issue date that's on your current license. That will not be the same as when you got your first license if you have renewed since you got your original license.&lt;BR /&gt;&lt;/P&gt;</description>

      <description>&lt;P&gt;New Florida licenses do not have an issue date. I'm still trying to find the work around.&lt;/P&gt;</description>

      <description>&lt;P&gt;Leave ALL the data blank, no license number, no date.&amp;nbsp;&lt;/P&gt;

&lt;P&gt;The license information is not required. The filing will continue without the information, but will show an error if you only enter the number and not the issue date.&lt;/P&gt;</description>
