When you fill out the Form 9465, Installment Agreement Request, you choose the date in which the payment is to be made. This is shown on Line 12.

If you applied for an installment agreement using TurboTax, you can find this form by taking the steps below.

Sign into TurboTax with the same account you used to file your return.

Click on My TurboTax > Tax Timeline.

In the Some things you can do section, click on Download All Forms & Worksheets. Form 9465 will be within these pages.
