I know I have to download the file from the IRS website and fill it out myself, but is it possible to combine this file with the complete file I get from TurboTax and have it be filed together?

The only way you can attach this Form to your return would be to file it by mail through TurboTax. Unfortunately, there is no way to access this form through TurboTax at all.

EDIT:: You can mail this form into the IRS as well, if you file the return electronically.

You'll need to submit the entire return by mail, actually. I did a little more research and found that you can't file electronically if you need to submit some of the return by mail.
