You may be referring to Form 14039. It is an Identity Theft Affidavit. You would be required to fill it out if you selected anything that may have indicated that you have an identity theft issue.

If this form was selected by accident, then you can delete the form by doing the following:

Log into your account; click on Take Me to My Return if it doesn't come up automatically
Next, click on My Account on the top of the screen
Then click on Tools and select Delete a Form
Scroll through the list of forms and when you find Form 14039, click Delete next to it
It will confirm that you want to delete the form, hit yes (or continue)
The form will be deleted
