I live in CA & have filed my taxes electronically. I'm getting a refund that I applied to 2018. Do I have to send in form 8453-OL? How do I do that? Thanks

Used Delux turbo tax. When I printed out my taxes to have for my record the front sheet says to sign and date form 8453-OL within one day of acceptance. I didn't do that and the e-mail I received back from the government says I am all done. Do I have to do this form and if so how do I sign it and where do I mail it?
