To generate the Form IT-258, follow these steps:

While in your state return, look for a page titled Take a look at New York credits and taxes.
Scroll to the Your Home section (see attached).
Click Start to the right of Nursing Home Assessment Paid with Your Own Funds.

This form is not available in the TurboTax 2020 CD Windows version. I followed these instructions with a live tech support person and she could not locate it either.

Where is it and how do I file for it??
