I file our taxes jointly. My wife has a family HSA plan covering our family. I have been employed for only part of that last tax year during which I was covered by my wife's family plan as well as an individual HSA plan.

When selecting coverage, should I consider myself as covered by the family plan at all times or family plan for the months that I was not employed and self plan when I was employed (but still covered by both)?

Your spouse has a family plan and thus can make the maximum family contribution to her HSA. For her, select family coverage for every month. Unless the last month rule was used in 2024, do not answer the questions about 2024 coverage. You apparently do not have an HSA so there is no form for you to fill out.

This is not clear to me. When specifying what months we each had HSA health plan coverage from an HDHP, the options are given for each month are:

None

Family Plan

Self Plan

My wife had a plan all year (and December of 2024) that covered our whole family. I had a plan that covered me in December 2024 as well as part of 2025 (thus being covered by both for part of the year, and only my wife's family plan for the other part of the year).

As such, it appears as though I should select "Family Plan" for my wife for all months. For me, it appears as though I have 3 options. I'm trying to clarify the correct route. My considerations for me are:

OPTION 1: "Self Plan" for months that I had my own coverage, and "Family Plan" for the remainder

OPTION 2: "Self Plan" for months that I had my own coverage, and "None" for months that I was only under my wife's family plan

OPTION 3: "Family Plan" for all months as I was covered by her plan the entire time (in addition to the months that I had my own)

What's the correct thing to do here?

@Mike9241 any thoughts?

You were covered by a family plan all year, so that is the most logical option. Your self plan may be considered a backup to the family plan.
