State & State ID number missing on form W2G

I had winnings from an online casino in Nevada & received my form W2G. I live in Michigan & will need to report this to the state. Upon receiving my W2G, there is nothing in Box 13-State & the box with the payer's state id - as Nevada does not have state income tax. I am wondering what to do as I cannot proceed until I have something entered into these boxes. What do I do?

Leave boxes 13, 14 and 15 blank (empty) on the TurboTax W-2G.

There is an amount in box 14 though.....that's where I am stuck

@Heikki Since there are no state income taxes withheld in box 15, there is no reason to have anything in box 14
