Abbreviated 1099-R Information doesn't work

I withdrew the bulk of my Retirement Account. I received a very simple 1099-R, which has Box 1, Box 2a with the same information, an X in Box 2B, and the Federal Income Tax withheld in Table 4. Distribution code in Line 7 has 7, IRA/SEP/Simple has an X. The rest of the form is blank, or shows zeros. Trying to follow directions in TurboTax Desktop doesn't work.

Leave all the rest of the boxes Blank. Don't type zeros. Do you have anything in the State boxes 14-16? If not leave them blank. If you have state withholding in box 14 then enter the same amount from box 2a into box 16. If box 2a is blank use the amount from box 1. Which box is checked for 2b?

What exactly "doesn't work"? What screen are you on when it doesn't work? What happens when you make an entry or click a button on that screen? If you get an error message, what is the complete exact message?
