Third Party Sick Pay

Employee paid policy. Not noted on W-2. Received a seperate letter from said 3rd Party provider, but no Form number noted on what resembles a W-2. Just references income made while sick, Federal, Social Security, and Medicare taxes withheld. Where would I file this on my tax return? I would beleve this to be Income, but not sure where to note it.
