blank box 15 state employer ID on your W-2&lt;/B&gt;
  &lt;SPAN&gt; should not cause a problem with your return
processing at the IRS or the state revenue office. If there are &lt;B&gt;no entries
in 17&lt;/B&gt;, you may leave the &lt;B&gt;entire box 15 and 16 blank&lt;/B&gt;, as well.&amp;nbsp;
If you &lt;B&gt;do&lt;/B&gt; have entries in box 17, please see the work around below.&amp;nbsp; Note: Utah requires 11 numbers and then 3 capital letters at the end. &lt;BR /&gt;&lt;/SPAN&gt;
