if I have to use form 4852 employee, and I’m filing married-jointly for both filing federally and state in Colorado what all do I need to send in if doing it by mail.

You will need to send all official tax forms and schedules, and copies of all W2s and any 1099s that show withholding. For Colorado, it is the same- the W2s and any 1099s that have withholding.
