When I go to check out I am getting a 1099-DIV form that is blank except my name. It is asking me to enter zip code. Not sure how to remove this DIV form.

To delete your Form 1099-DIV, you can do the following:

For TurboTax Desktop:

Open/continue your return
Click on "Search"
And in the search bar type"1099-DIV"
Select "Jump to 1099-DIV"
It'll take you to a screen that shows your Form(s) 1099-DIV that you have entered.
Next to the Form 1099-DIV you want to delete, click on the (down-Facing) caret
Click on the "garbage can" to delete this form

For TurboTax Online:

Open/continue your return
Click on magnifying glass for Search in the top right of your screen
And in the search bar type"1099-DIV"
Select "Jump to 1099-DIV"
It will take you to a screen that shows your Form(s) 1099-DIV that you have entered.
Next to the Form 1099-DIV you want to delete,
Click on the "garbage can" to delete this form

Your screens will look something like this:

Thanks for your reply. Unfortunately there is no 1099-DIV besides the one that is completely filled out correctly. The 1099-DIV it shows me when I run the check is completely empty.

You have a form that you can't see and can't delete, time to contact support.
