I contribute to my 401k plan through my employer. Does the retirement plan box 13 on the w-2 have to be check off?

Yes.

If you contribute to a 401(k) plan at work, box 13 - Retirement plan on your form W-2 has to be checked.

And the amount that you contributed to your 401(k) is reported in box 12 Code D.
